Hauora Cornerstone Support Programme

Hauora Cornerstone Support Programme

Compass Health Māori and Population Health team is excited to offer a co-ordinated and comprehensive support programme to assist general practice teams as they undertake the Treaty of Waitangi and cultural competencies in Cornerstone accreditation.

To coincide with Matariki and Te Wiki o Te Reo Māori, The Hauora Cornerstone Support programme was launched at the Takapuwahia Marae on the 24th July. The people of Ngati Toa graciously opened their marae to Compass Health as they welcomed all in attendance with a traditional powhiri.

The programme is a combination of structured support from the Māori Health Team along with all the relevant resources to support your practice to meet the standards.

If you would like to find out more about the programme and register your practice please email maorihealth@compasshealth.org.nz.

All resources are also available on the practice Provider Portal.

Services for Whānau - Outreach Nursing

Outreach nurses help people who have difficulty accessing their GP or practice nurse for reasons such as cost, transport, language, and/or cultural barriers. Outreach nurses can provide assistance for GP visits, transport to appointments and medications. They also work with other agencies such as Work and Income and Child, Youth and Family to ensure other social needs are being met.

Our Outreach Nurses are a mobile service, and they can meet with you where you are most comfortable to discuss your health issues. They can help you with:

  • cervical smears;
  • cardiovascular risk assessments (also known as heart checks);
  • mammograms;
  • diabetes annual reviews and testing;
  • blood tests;
  • immunisations; and
  • health checks.
Accessing the Service

The Outreach nursing service is available to all Māori patients who are cared for by one of Compass Health's practices. The service is also available for those patient's that don't yet have a GP, but live in the region. You can access the services through your general practice.
