What is a Stanford Self-Management Course? 

The Stanford programme is based on over 25 years of research and is used in 25+ countries around the world. Whatever health issues you have, this 7 week course (week one being the introductory session) can help as you learn a range of generic skills and tips to maximise your health and wellbeing.   

The Stanford self-management programme is also known as:

  • My Health, Our LIFE
  • LIFE Programme 
  • Stanford Model
Developed by Stanford University the Stanford Chronic Disease Self-Management Programme (CDSMP) is an evidence-based programme that has been adapted for many cultures and found to be equally useful.    

The course consists of two and a half hour sessions, for six or seven weeks. These are held during the day, evening or weekend to suit local groups and are held in meeting rooms, clinics, community centres, churches and marae.   

The sessions are suitable for anyone with an ongoing health condition, or risk of health issues including prediabetes, high blood pressure and obesity. It is designed for people with different ongoing health problems (diabetes, arthritis, asthma, depression and much more) to attend together and your family or carers are welcome to join you. They will learn lots too!    

Workshops are facilitated by two trained leaders, one or both of whom are peer leaders (non-health professionals) with long-term health conditions themselves. This means they really know what it is like to live with the day to day challenges some health conditions bring.   

Subjects covered in these workshops include: 

  • ways to deal with problems such as frustration, fatigue, pain and isolation 
  • safe exercise for maintaining and improving strength, flexibility, and endurance 
  • appropriate use of medications 
  • communicating effectively with family, friends, and health professionals 
  • nutrition 
  • shared decision making, and 
  • how to evaluate new treatments for yourself.

Diabetes: Your Life Your Journey

Diabetes: Your Life Your Journey was developed locally as a research project six years ago which produced the evidence that it is valuable for the health of people with Type 2 Diabetes and their Whanau/families. Since then it has continued to be a successful programme run by Compass Health and many courses have been facilitated in the community. 

The programme consists of two hour sessions once a week over a six week period. Some of these are held during the day and some in the evening at various venues across Wellington, Porirua and Kapiti, including Marae and community venues. The facilitators are Diabetes Nurses and/or a Dietitian.

Diabetes: Your Life Your Journey will help you learn some tricks and tips to manage your diabetes effectively and maintain your health and wellbeing.  At the end of the six weeks you should feel confident in your ability to control your diabetes and have the skills, or know where you can access help, to achieve good diabetes control.

We encourage you to bring a member of your family or friend so they can help support you with your diabetes.. They will learn lots as well!
